Patient Information

From the moment you enter our facility you will feel welcomed.  Our knowledgeable, friendly staff looks forward to meeting you and helping to decrease your pain, improve your mobility, restore your function and overall well-being.

You will find our office environment very compassionate, fun and family oriented.

Physical therapy visits are by appointment only.  Please call and schedule your initial visit.

Initial Visit

 

What can I expect during my initial visit?

  • At the initial visit the physical therapist will evaluate and interview you to discuss your medical history and the current injury or source of pain.  As well as find out your goals and expectations following your rehab. Your physical therapist will also evaluate your injury or condition using specific, diagnostic tests and assessments. Your physical therapist will create a treatment plan specific to your individual needs and discuss it with you, as well as providing written information such as a home exercise plan to complement your in-office treatments.

What do I need to bring to my first visit? 

  • Be prepared to spend 1-1.5 hrs during the initial visit.
  • Wear comfortable, loose-fitting clothing.
  • Please arrive 15 minutes early to fill out paperwork.  You can also download and fill out most forms prior to arrival.
  • Bring your physical therapy order from your doctor (if they haven’t faxed it already)
  • Bring your insurance card(s) and a photo I.D.
  • Often a copayment or deductible is due at initial visit and subsequent visits.  We will notify you of payments due on the initial visit.  If needed, we can arrange a payment plan.
  • Follow up visits usually last 45 minutes to one hour.
